This door sign reads MEDICAL RECORDS in English and REKOD PERUBATAN in Bahasa Malaysia, with a Chinese version (病历室) available. It marks the room where patient files and archived records are kept in a GP clinic, panel clinic, specialist centre, hospital medical records department or occupational health unit in Malaysia. Secure, clearly identified record storage is part of what private healthcare facilities are expected to maintain, and a door sign helps staff and auditors find it. The design is a landscape 3:1 door plate in navy acrylic-look style with a files pictogram. Stick it on the door at eye level or on the wall beside the frame; add our Staff Only or Keep Door Closed plates where patient confidentiality requires restricted access.
